The applicant or leader of the group needing to use the church facilities and/or Media Department for their audio, stage, video and lighting needs must fill out this form and submit it to Church Office at least (3) weeks* before the activity is scheduled. The applicant will be contacted after this form has been reviewed. Please keep a copy with you at your activity.

* Exceptions will be made for authorized activities that have been scheduled at shorter notice. ** Bear in mind that it will not always be convenient for someone to open or close the facilities. Please be on time. NOTE: We cannot be responsible for anything you leave behind. Make sure you check the areas you used for personal belongings or items that need to go in the trash.

Now that you have completed your Facility Request Form, if you also need Media at your Event, please fill out the Media Request Form below.

This Media Form must be completed only if media support is required or emailed to djohnson@vccenter.net. You should receive an email or telephone response within one week of your request. If you do not receive a response, please call David Johnson at (704) 602-6032. Please do not contact individual crew members about your event.
PLEASE SUBMIT FORM AT LEAST 3 WEEKS IN ADVANCE OF THE EVENT.


SPECIAL NOTATIONS FOR RECORDINGS

  • Songs used for playback can be on a MP3 player or CD. If using a MP3 player a playlist should be created with the songs in the order in which they will be played. If using CD, the CD should be properly labeled with the TRACK#, TITLE and LENGTH OF SONG for each song. Songs should be in the order in which they will be played.

  • You must supply the music you desire for your event. All background music should be recorded onto one CD.

  • Please submit music 2 weeks in advance for approval of recording quality.
