When entering badge information, please make sure each name is placed in the correct section:
Pocket Badge: Enter the name(s) exactly as they should appear on the badge.
Magnetic Badge: Enter the name(s) exactly as they should appear on the badge.
If you are ordering both pocket and magnetic badges, please check both boxes and enter the appropriate name(s) in each section.
Badge Pricing
Magnetic Badge: $8.00 each
Pocket Badge: $9.00 each
Important: Please double-check the spelling of all names and ensure each name is entered under the correct badge section before submitting the form. Badges will be created exactly as the information is provided.
Payment: Please submit all funds to the Welcome Center in a sealed envelope clearly labeled with the specific Ministry of Helps (MOH) area the payment is for.
